  Admissions:
Who is eligible to receive services?

1. Those with a developmental disability (typically with a diagnosis of mental retardation) or other special need such as mental illness or brain injury.
2. Those who have approval from a funding source (federal, state, county, or private). Click on CPC (Central Point of Coordination list or DHS (Department of Human Services) lists (see link under # 2 below).
3. Those who have completed the entire admission process.
4. Those who do not require 24-hour nursing assistance.

How do I apply?
1. Contact any of the COC offices to schedule a tour.

2. Contact your CPC for funding approval:

3. Complete and mail in the COC application.
Our staff will contact you after the information is received and when an opening occurs.
