COC's purpose is to provide people with disabilities and
other special needs the opportunity to reach their potential, but another very
important aspect is the one of having relationships with others. In addition,
we challenge the staff persons who provide the supports to look at the living
arrangement in terms of:
"Is
this the kind of place where I would want to
live?" |

Each
person supported at COC decides with his team what living arrangement best
suits his or her needs. Each person is encouraged to be as independent as
possible in taking care of themselves and their home. Individual rooms express
the tastes and preferences of the occupants. Staff persons, called Living
Skills Advisors at COC, are encouraged to be a partner, not a parent, and to be
a supporter, not a server. Staff persons also provide guidance and support with
a Christian emphasis in accordance with COC's values. They give opportunity for
devotional times and facilitate attendance at local churches, including worship
services and Bible study groups.
Depending on the program, various other support services, such as
physical therapy, occupational therapy, speech therapy, recreational therapy
and nursing services, among others, may be available. People supported are
encouraged to become included in their communities by attending church at local
churches, joining in various recreational opportunities, and by forming
relationships with other members of the community. |

ICF/MR (Intermediate Care Facility for Persons with
Mental Retardation) The ICF/MR programs provide intense supports and
training to persons with severe disabilities who need 24-hour supervision and
support, while adhering to the principle of active treatment. The program is
highly regulated and supports people with significant intellectual
disabilities.
COC has ICF/MR programs in its Pella/Knoxville and
Indianola/Des Moines regions. In Pella, we support 23 people in three homes. In
Indianola, 16 people are supported in two ICF/MR homes. |
HCBS (Home and
Community Based Services) The HCBS programs provide training and
support to people living in shared homes in the community with opportunities
for learning skills to help develop independence. The HCBS homes are
self-contained homes, which include living areas, laundry facilities, and
bathrooms, and the people supported who live there share these common living
areas, though they have their own individual rooms COC has HCBS homes in its
Pella/Knoxville and Indianola/Des Moines regions. In Pella, the HCBS program
serves 21 people in 5 homes. One of these homes is designated a "retirement"
home, and it is staffed to allow for greater support for the aging residents.
In Indianola, the HCBS program serves 17 people living in 4 homes and we now
serve 7 people in 2 HCBS/MR homes in Des Moines.

BA-SCL (Broadway Apartments-Supported Community
Living) At the Broadway Apartments, 15 adult individuals are able to
live in their own apartments - three in single apartment settings, & 12 in
double-occupancy apartments. These people need to be able to complete many
daily tasks with minimal staff support. One staff person is available in the
evenings & on the weekends to offer some assistance with tasks such as
cooking, laundry, cleaning, & arranging transportation. A staff person is
also in the building throughout the night. |
SCL (Supported
Community Living) The SCL program is for those people who just need
someone to "drop-in" to assist them with various things. Individuals in this
program can live with their families, or they can live in their own home or
apartment. A Living Skills Advisor will schedule time to meet with them every
week to assist them with needed tasks - budgeting, cooking, cleaning, shopping,
appointments, social skills, problem solving, etc.
RESPITE
This is a program designed to provide support for individuals, typically
children, to give a break to their regular caregivers. Living Skills Advisors
will spend 1:1 time with the individual so that their caregiver is able to
complete errands, clean the house, or just take some needed time for
themselves. This program is available to both children & adults.
